How to Order

• Click on the Music button at the left to select music you want to order.
   
• Print the order form below. You will need to have Adobe Acrobat Reader installed on your computer. Click on the Acrobat Reader icon at the bottom of this page to obtain this free software.
   
• Fill out the order form. Please be sure to add appropriate tax if shipment is to be made to Pennsylvania.
   
• If your purchase is for a tax-exempt organization in Pennsylvania (a church, for example), please enclose a completed Pennsylvania exemption certificate.
   
• Add a charge for shipping and handling:
   
  • $4.00 for orders of scores totaling $40 or less.
  • 10% of the merchandise total for orders over $40.
  • Express shipping from FedEx is possible if all scores are in stock.  If you request express shipping and we can honor your request, you will be billed separately for the additional shipping costs.
     
• Be sure to enter your e-mail address on the order form. This will facilitate acknowledging your order and handling any problems that may arise.
   
• Total your order, and send the form with a check or money order payable to Doug Starr to the address listed on the form.
   
• Remittances must be in US funds, drawn on a US bank or an international branch thereof.
   
• Note that we do not accept credit cards at this time.

If you have difficulty printing the order form from Acrobat Reader—different printers will print the image on the page differently—try altering the settings in the Copies and Adjustments section of the Acrobat Reader Print dialog box.
